Patient received 5th in the anthrax series in 2004. Patient then received a 6th dose in Jan 2025. The injection site is red, hot to touch with hardness of tissue projecting out 2-3 inches from injection site. Approximately the size of a racquetball is the area where inflammation and pain exist. Lifting the arm is possible; however, at 90 degree raised elbow there is soreness. Above 90 degrees pain exists.

pt says her arm bled after the getting the injection. She went home and took a nap. When she woke up she thought she was having a stroke. Her head was hurting and her eyes were burning. She was treating it was like a sinus headache/infection. After a few days her left eye was still hurting, so she went to eye Dr. He did an eye scan and asked some questions. He told her he thought it maybe symtoms of the Shingles vaccines. He prescribed her 12 days of Valtrex. After about 3 days of taking this medication she lost vision in her left eye. After about 2 days it cleared up and she was able to see again. She followed up with him again and she was better at this time. On Jan 10th she went to the Dentist with jaw pain. On Jan 17th she went to urgent care with fluid on the left ear. She was given antibiotics to take for 10 days. After taking the meds for 8 days she was any better. She callled her PCP. She was treated and prescribed levofloxacin. After about 3 days the medicine started to help the left ear. The left side of her face was itching around her eye and around her waist on her torso. She was cold, shaking, and feeling bad wanting to sleep alot. She went back to her PCP. The left ear was still red and was told to complete the medicine given at the last appt. She says she is a little better but is still weak and still itching and when she scratches it is painful but her headache finally went away.

Patient in clinic for immunization. Patient has syncopal episode after his final vaccine. Patient was diaphoretic, pale and was not able to respond immediately after receiving final vaccine. The nurse called for assistance and used an ammonia towelette. After just a few seconds the patient came to and began to speak and started to respond. At 11:05 BP was 60/40, The patient given wet paper towel, and a bottle of water, attempted to raise feet above head but patient refused. At 11:10 BP 98/59, P 70, R 24. After a few minutes we were able to prop his feet in a chair. A fan was placed beside the patient’s face. 11:15 BP 107/70, P 62, R 16 and 11:20 BP 99/69, P 65, R 16. SEE CONTINUATION. The patient remained in office x 20 minutes. At 11:25 BP was 106/71, P 64 and the patient’s color returned to normal, he was no longer diaphoretic. The patient was ambulatory and left without any assistance. Notified RN CDNS, RN PCM, DM and RD
